Case Study 4
A Well Wise Hub Case Study
Patient profile: Female, 67 years old.
At Well Wise Hub, we support people who have been living with long-term health conditions and feel stuck, tired, or overwhelmed. Even after many years, positive change is still possible.
This case shows how coordinated care, lifestyle guidance, and personalised support helped a 67-year-old woman improve her health, safety, and daily comfort.
The Client Story
Our client had been living with Type 2 diabetes for over 12 years. Despite taking medication, her blood sugar levels remained high.
She was also experiencing ongoing nerve pain in her feet, reduced kidney function, increased heart risk, episodes of low blood sugar, and low confidence about her health.
She felt frustrated and worried about future complications. She wanted better control and a clearer plan.
Our Coordinated Approach
At Well Wise Hub, we work alongside GPs and specialist teams to ensure safe, professional, and integrated care.
After a full health and lifestyle review, we built a structured support plan focused on improving blood sugar balance, reducing nerve discomfort, protecting kidney and heart health, supporting safe medication review, and enhancing strength and independence.
1. Collaborative Medication Review
We encouraged discussion with her specialist team to ensure her medication plan was safe, effective, and appropriate for her stage of health.
This reduced the risk of sudden low blood sugar episodes and supported better long-term management.
2. Personalised Nutrition Strategy
We introduced a simple, portion-controlled eating approach focused on balanced meals, lower-sugar carbohydrate choices, adequate protein to protect muscle strength, heart-friendly fats, and sustainable weight management.
The goal was steady improvement, not extreme dieting.
3. Well Wise Hub Supplement Therapy
As part of our integrative model, we provided personalised supplement therapy aligned with her health needs and coordinated with her medical team.
Our supplement therapy and treatment techniques were individualised, carefully monitored, designed to support metabolic balance, and focused on nerve and overall wellbeing support.
This formed part of a broader whole-person strategy.
4. Nerve Pain and Foot Care Support
We included structured nerve-support techniques, gentle therapeutic modalities, education on daily foot care, and referral pathways where needed.
This reduced discomfort and improved daily confidence in walking and mobility.
5. Movement, Strength and Sleep Support
To protect independence, we introduced supervised strength-building exercises, light aerobic activity, balance and mobility work, and sleep improvement techniques.
This helped preserve muscle strength and improve energy levels.
Outcomes at 12 Months
After one year of structured support, blood sugar levels improved significantly, nerve pain reduced, kidney health stabilised, weight reduced modestly, low blood sugar episodes became less frequent, and daily confidence and mobility improved.
She reported feeling safer, stronger, and more in control of her condition.
Why This Matters
In long-standing diabetes, success is not just about numbers. It is about preventing complications, protecting independence, reducing hospital admissions, improving daily comfort, and supporting safe medication management.
This case shows that even after many years, structured and coordinated support can deliver meaningful improvement.
